As the authorisation person submitting this form, I have checked with all players listed on this entry form and they all agree to the guidelines and conditions of the COVID-19 Safety plans as may be set by the NTB&SA and abide by  all directions of the Club's authorised personal  for COVID-19 Safety plans and all Government conditions.

As the authorisation person submitting the form, I have check with the Club I have nominated the team for and that they are aware I have entered this team and the players as listed.

New players and aware the Player Registration fee is $30 per player (If have not registered in the 2025 Snooker season)

As the authorised person submitting the form all players are aware that they must be a financial member of the Club they are representing and if found to be not financial members the player/s will be ineligible to play.

The expected start date is Wednesday November 12th, depending on how many teams nominate (Wednesday and Thursday Nights plan for play) - Monday may be used if too many teams nominate.
