As the authorisation person submitting this form, I have checked with all players listed on this entry form and they all agree to the guidelines and conditions of any COVID-19 Safety plans as may be set by the NTEBA and abide by  all directions of the Hotel / Club authorised personal for any COVID-19 Safety plans and conditions including Government regulations

As the authorisation person submitting this form, I have check with the Hotel Club I have nominated the team for and that they are aware I have entered this team and the players as listed.

As the authorised person submitting this form for the team I have nominated, they know they will use Pool Stats system for scoring via the Live scoring system of Pool Stats

As the authorised person submitting this form, the individual team players I have listed or future individual team players are aware they may be required to use an electronic player registration form to sign to the association - further details will follow on this if required.

The Team  I have nominated are aware there will be no team affiliation fee payable for Hotels / Clubs and a fee of $25 per team / per night for this roster.

The team captain or nominated person is aware of the $25 per team / per night and it is their responsibility to collect the $25.

Please Note: Individual payments from players will not be accepted, the whole $25 must be deposited and from just one person (Captain or nominated person) - No individual payments

The captain or nominated person are responsible to deposit - Electronic transfer to the NTEBA bank account, payable to the NTEBA Treasurer

Sanctions will apply to teams that are more than 4 weeks behind = Loss of Points for the Match/s played if un-financial.
