Nominations can be submitted online, or download team registration sheet below and fax back to 63434417 or collect registration sheet from your local Hotel / Club (Forms will be sent to Hotles / Clubs by Sunday September 15th)

Format: Minimum 5 Player Per Team - 3 Rounds of 5 Singles - Starting at 7.15pm
